We request that patients pre-register for their appointments, scan insurance cards, and verify demographic information annually. It is important to confirm coverage information with your insurance plan, as some HMO plans may no longer be in network or may require a referral from a primary care physician. If you are changing insurance plans and are on a biologic or infused medication, please provide the front and back of your insurance card as soon as possible. Our office will need to initiate new prior authorizations, which may take additional time to process.
